Case Study: 9-year-old Quarter Horse Gelding with Right Forelimb Lameness
History: Presented for evaluation of right forelimb (RF) lameness noticed over the last 2 weeks.
Initial Evaluation: Objective lameness evaluation on asphalt and sand indicated a mild initial lameness in the RF, which resolved with exercise, making blocking potentially ineffective.
Physical Examination: Examination of the torso and limbs revealed no significant abnormalities.
Treatment Decision: Suggested CT examination, but owner opted for initial rest for 2 weeks followed by gradual return to exercise.
Follow-up Evaluation (2 Weeks Post-Rest): Mild to moderate RF lameness intermittently observed. Horse developed resistance during left forelimb trimming/shoeing.
Re-evaluation (1 Month Later): No measurable forelimb lameness detected, but horse continued to resist lifting and holding the left forelimb.
Diagnostic Imaging: Standing CT of forelimbs revealed a proximal P2 cyst extending into the pastern joint.
Treatment and Outcome: Owner chose intra-articular steroids for the pastern joint without improvement; lameness worsened.
Further Intervention: Right forelimb ring block above the pastern allowed lifting of the left forelimb, indicating localized pain.
Surgical Intervention: Under anesthesia, attempted cyst injection guided by radiography; cyst visibility was challenging.
Clinical Significance: CT proved essential for diagnosing a cyst not visible on standard radiographs. Blocking for lameness localization can be inconclusive.
